Passport
Passport, which must be valid and signed, and have to be valid at least 6 months beyond intended stay and must have at least one blank visa page.
You have to renew your Passport if it will not be valid at least 6 months beyond intended stay.
Photo
Two recent professional passport-type photographs.
Quality Requirements for Passport Book & Passport Card Photographs
Proof of U.S. Status
A copy of evidence of legal status in the U.S. (Alien Registration Card/Green Card or Valid Visa with I-94 in Passport)
Bank Statement
A copy of your recent bank statement or evidence of adequate finance, such as a letter from a bank. It is advisable to erase account numbers.
Hotel Confirmation
A copy of your hotel confirmation(s) is required if staying at a hotel.
Letter of Invitation
A letter of invitation in writing is required explaining all of the following:
- Intended dates of arrival and departure from the country
- Traveler’s name and Passport number
- Request of any special Visa conditions (Multiple Entries Visa) if required
- Financial support and return transportation for the applicant
The letter should be from host company or organization in South Africa.
Minors
A notarized consent letter from BOTH parents is required.
Financial Responsibility
A letter from the U.S Organization taking financial responsibility, and undertaking to return applicant upon completion of assignment if applicant is sent by Church Organization/Mission. This letter should also specify purpose and duration of stay in South Africa.
Police Clearance
A Police Clearance Certificate (criminal background check) of all countries the applicant lived in since the age of 18 for 12 months or longer.
Health Coverage
Proof of Health Coverage while staying in South Africa is required.
Repatriation Deposit
A $1500.00 money order for Repatriation Deposit lodged in a case an applicant travels on a one way ticket.
Proof of Departure
A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.
Medical Forms
One completed Medical Report Form, MUST be signed.

One completed Visa Application, MUST be signed.
WVT Work Order
You must fill in the WVT Work Order Form. Download here
The order form gives WVT your name, address, shipping information and payment method. Multiple applicants can be entered using one work order form. You only need to complete one WVT work order form per request. Please print two copies of the confirmation page. Include one copy with the documents you ship to WVT and keep one copy for your records.
You may pay our fees with a credit card or enclose a company/cashier check or money order.

Please use FedEx Priority Overnight. If you are going to use FedEx First Overnight, please let us know. Otherwise, this delivery method will delay the process.
