| Passport | Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages. Copy of bio-page of passport is needed. This copy must be NOTARIZED. |
| Photo | Two passport-type photographs. Quality Requirements for Passport Book & Passport Card Photographs |
| Proof of U.S. Status | A copy of evidence of legal status in the U.S. This copy must be NOTARIZED. |
| Letter of Invitation | A letter of invitation from host in South Africa. A copy of ID or passport is needed from the host. |
| Hotel Confirmation | A copy of your hotel confirmation(s). |
| Vaccinations | International Certificate of Vaccination for Yellow Fever. |
| Bank Statement | A copy of your recent bank statement or evidence of adequate finance, such as a letter from a bank. |
| Minors | A person under 18 years of age not traveling with both parents or legal guardian(s) must provide a letter of consent signed by the non-accompanying parent(s) or guardian. A copy of the minor’s birth certificate is also required and must be NOTARIZED. |
| Proof of Departure | A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent. |
| Application Form | Download Application Form One completed Visa Application, MUST be signed. Must be written in BLACK Ink. |
| WVT Work Order | Please complete WVT Work Order Form. Download here Payment can be processed with a credit card or a company/cashier check or money order. |
