Required Documents  
Passport Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages.
Photo Two passport-type photographs. Quality Requirements for Passport Book & Passport Card Photographs
Proof of U.S. Status A copy of evidence of legal status in the U.S.
Letter of Invitation For citizens of Israel and Turkey, applying for a 6 and 12 months Visa, a letter of invitation from host or organization is required.
For citizens of all other countries, but those mentioned above, a letter of invitation from the Bureau for Migration and
Asym of the Ministry of Internal Affairs of the Republic of Moldova is required for all types of Visa.
Proof of Departure A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.
