| Passport |
U.S. Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages.

| Photo |
Passport-type photographs.To submit a passport photo, please follow these guidelines:
- The file must be in JPG or JPEG format.
- The file size should not exceed 1MB.
- The photo must have been taken within the last 6 months.
- It should not be the same photo currently in your passport.
- The height and width of the photo must be equal.
- The background should be plain, light-colored, or white.
- Your head should be centered within the frame.
- The photo should show your full face, from the top of your hair to the bottom of your chin.
- You should be facing the camera directly, with your eyes open.
- There should be no shadows on your face or the background.
- Do not wear glasses in the photo.
- The photo should not have any borders.

A copy of your Driver’s License, Major Utility Bill (Water, Gas, Electric, etc.), or a copy of valid lease agreement. |
| Business Letter |
A business letter from the company is required. Download Business Letter

| Letter of Incorporation |
A Letter of Incorporation from the host company in India. |
| Minors |
Parental Authorization Form for minors travelers: Download Parental Authorization Form
One completed Form, MUST be notarized and signed. Photocopy of a Birth Certificate is required. |
| Applicants of Indian Origin and Dependent Family Members |
Persons of Indian origin are required to surrender Indian citizenship.
A copy of a renunciation certificate AND naturalization certificate.
Persons of Indian origin who have NOT renounced Indian citizenship or
CANNOT SHOW PROOF of renunciation must first apply for renunciation directly through CKGS:
