Required Documents  
Passport Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages.
Photo One passport-type photographs. Quality Requirements for Passport Book & Passport Card Photographs
Proof of U.S. Status A copy of evidence of legal status in the U.S.
Vaccinations International Certificate of Vaccination for Yellow Fever.
Proof of Departure A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.

One completed Visa Application, MUST be signed. Application can be filled electronically or with Black INK only, in BLOCK / CAPITAL letters.

Hotel Confirmation/ Letter of Invitation A copy of the hotel confirmation or
If the traveler is staying in a private home, a letter of invitation from the host is required.
If the traveler host is a Ghanaian Citizen, a copy of his/her Ghanaian Passport/Ghanaian ID is required.
If the traveler’s host is not a Ghanaian Citizen, a copy of his/her passport and his/her Ghana visa/resident permits is required
