Required Documents  
Passport U.S. Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages.

Photo One passport-type photographs. Quality Requirements for Passport Book & Passport Card Photographs
Letter of Invitation A notarized letter of invitation from private host/tourism agency in Democratic Republic of Congo
Vaccinations International Certificate of Vaccination for Yellow Fever.
Proof of Departure A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.

One completed Visa Applications, MUST be signed.
Bank Statement A copy of your recent bank statement or evidence of adequate finance, such as a letter from a bank.
(need to provide this if you don’t have a notarized invitation letter from private host/tourism agency in Democratic Republic of Congo)
