Required Documents
Passport U.S. Passport – have to be valid at least 6 months beyond intended stay, signed and must have at least two blank visa pages.
For Additional Visa Pages and U.S. Passport Renewal.
Photo Two passport-type photographs. Quality Requirements for Passport Book & Passport Card Photographs
Letter of Invitation A copy of your hotel confirmation(s)/notarized invitation letter from host in Algeria( family or friends), the host must provide Algerian ID or Algerian passport/certificate from a travel agency specifying the nature and place of trip 
Hotel Confirmation A copy of your hotel confirmation(s).
Proof of Departure A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.
