| Letter of Invitation |
A letter of invitation from a Ukrainian tourist agency or from a Ukrainian hotel. |
| Private Visit Purpose |
For a Home Stay/Private Visit Visa – an original invitation issued in the name of the applicant
from a local Immigration Authorities of the Ministry of Interior of Ukraine is required.For Medical Treatment – an official invitation
from the Ukrainian medical institution providing treatment to the applicant. |
| Proof of Departure |
A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent. |
