Passport Copy of U.S. Passport – have to be valid at least 6 months beyond intended stay and must have at least two blank visa pages.

Photo Passport-type photographs.  Quality Requirements for Passport Book & Passport Card Photographs

To submit a passport photo, please follow these guidelines:

  • The file must be in JPG or JPEG format.
  • The file size should not exceed 1MB.
  • The photo must have been taken within the last 6 months.
  • It should not be the same photo currently in your passport.
  • The height and width of the photo must be equal.
  • The background should be plain, light-colored, or white.
  • Your head should be centered within the frame.
  • The photo should show your full face, from the top of your hair to the bottom of your chin.
  • You should be facing the camera directly, with your eyes open.
  • There should be no shadows on your face or the background.
  • Do not wear glasses in the photo.
  • The photo should not have any borders.

One completed Form, MUST be notarized and signed. Photocopy of a Birth Certificate is required.
Applicants of Indian Origin and Dependent Family Members Persons of Indian origin are required to surrender Indian citizenship.
A copy of a renunciation certificate AND naturalization certificate.
Persons of Indian origin who have NOT renounced Indian citizenship or
CANNOT SHOW PROOF of renunciation must first apply for renunciation directly through CKGS:

Proof of Departure A copy of your round trip airline tickets, or itinerary, or letter of confirmation from a travel agent.
